MESSAGE
On 10 March 2008, at approximately 1820 hours, inmate Omar Bleechington Spin#137840, was "touch stunned" with a taser by Lt. Leod Davis while in the Leon County Jail. Mr. Bleechington was being housed in adult "G" pod room #34 at the time of this incident. Reportedly, Mr. Bleechington would not obey verbal commands given by Correctional Officer Richard Smith and Lt. Leod Davis to remove his arm from the "food flap" located in the cell door.
On 11 March 2008 at 1133 hours, the sheriffs office was initially made aware of the incident
when Anabelle Dias, Omar Bleechington's defense attorney, contacted records supervisor Henry
Eggers via e-mail and requested copies of all reports! documents in regards to the incident. Mr.
Eggers then notified his chain of command about the incident and of Ms. Dias' request.
On 12 March 2008, Captain Brent Coughlin notified me of the allegation and asked me to handle the investigation. Capt. Coughlin related that he had communicated with Anabel Dias via e-mail regarding her concerns and inquiries. Ms. Dias was requesting an internal investigation into the above incident. Capt. Coughlin advised Ms. Dias that I would be handling the investigation and there were proper procedures that had to be followed.
On 17 March 2008, I responded to the Leon County Jail and met with Capt. Jackie Martin who provided me with the following:
1. A copy of recorded video of the incident on 10 March 2008.
2. Copy of Lt. Leod Davis' offense report, Response to Resistance Report and LCSO advanced taser user report.
3. Sgt. Brad Hutchinson's continuation and Response to Resistance Report.
4. C.O. William Dunn's continuation and Response to Resistance Report.
5. C.O. Richard Smith's continuation and Response to Resistance Report.
6. C.O. Anthony Revell's continuation and Response to Resistance Report.
7. C.O. Jason Tadder's continuation and Response to Resistance Report.
8. Medical reports from 10 March 2008 thru 11 March 2008 pertaining to inmate Omar Bleechington
